המחטף המגדרי של ניצן הורוביץ: רגע לפני הבחירות, משרד הבריאות מפרסם טיוטת חוזר המבקשת לחולל שינויים דרמטיים בנוגע לתהליכי שינוי מין. בין השינויים הבולטים: שינוי המין במרשם האוכלוסין (תעודת זהות) רק על סמך הצהרה, ללא צורך בטיפול רפואי; ומימון ציבורי לניתוחי שינוי מין, לרבות ניתוחי הסרת חזה לקטינים (בהסכמת הוריהם).

מדובר בחוזר של חטיבת הרפואה במשרד הבריאות, המופנה לכלל מנהלי בתי החולים ומנהלי אגפי הרפואה בקופות החולים. על החוזר חתומה ד"ר הגר מזרחי, ראש חטיבת רפואה במשרד. החוזר פורסם ביום שלישי האחרון, וניתן להגיב לו עד ה-16 בנובמבר – כלומר לא מן הנמנע כי תוכן החוזר ייושם בפועל עוד בטרם תוקם ממשלה חדשה. עשר אגורות (2¢):
בארה"ב באופנה לדבר על "טיפול המאשר מגדר", לקטינים הרואים עצמם כטרנס-מגדריים. הם כבר לא מדברים על סתם ״טיפול".

בתור פסיכותרפיסט אני בעד "טיפול". אני פשוט טוען שהטיפול צריך להתחיל בפסיכותרפיה, ולא בחלוקת מרשמים מיידי להחלפת הורמונים ותזמון של ניתוחים בלתי הפיכים.

למרות שישראל נראת מאופקת יותר מכמה מדינות בארה״ב, שר הבריאות היוצא ניצן הורוביץ עדיין מנסה לקדם את האג'נדה השמאלנית, שלדעתי בריאות הנפשית של מתבגרים לא נמצאת בראש סדר העדיפויות שלה.

מפלגת מר״צ של הורוביץ הפסידה מנדטים. אז הוא לא יחזור לכנסת בקדנציה הקרובה. הקואליציה השמאלנית שלו תשב באופוזיציה. לכן, זה לא מפתיע שהוא ממהר בניסיון בקומבינה אחרון להשפיע ולהפוך את כל מה שהוא יכול.

ברגע שאני שמעתי את הביטוי "הטיפול המאשר מגדר", אני חשבתי על מצב רפואי הזה:

גינקומסטיה (המכונה לפעמים "ציצי גבר") היא מצב שכיח הגורם לשדיים של בנים וגברים להתנפח ולהיות גדולים מהרגיל. זה נפוץ ביותר אצל נערים מתבגרים וגברים מבוגרים...

גיל ההתבגרות

במהלך ההתבגרות, רמות ההורמונים של בנים משתנות. אם רמת הטסטוסטרון יורדת, אסטרוגן יכול לגרום לרקמת השד לגדול. לבנים רבים בגיל ההתבגרות יש מידה מסוימת של הגדלת חזה. גניקומסטיה בגיל ההתבגרות בדרך כלל מתבהרת ככל שבנים מתבגרים ורמות ההורמונים שלהם הופכות ליציבות יותר...

אפשרויות הטיפול כוללות

• תרופות להתאמת חוסר איזון הורמונלי
• ניתוח להסרת רקמת השד העודפת 

מובן לי שלעיתים אפשר לטפל בגניקומסטיה באמצעות שאיבת שומן בלבד, לרוב הליך פחות מסובך מהסרת רקמת השד ושינוי צורת הפטמה בנשים ביולוגיות, הבוחרות לעשות כך. העור של נער יכול להיות גמיש, וכאשר מתפתחים שרירי החזה של נער, ייתכן שלבחור הסובל מזה טיפול נוסף לא יצטרך.

גינקומסטיה עלולה להיות הרסנית לבנים, אם הם לא עוברים טיפול. נערים רבים פשוט מנסים להסתיר את מצבם, מפוחדים שהם יצטרכו להוריד את החולצות שלהם במהלך שיעור חינוך גופני לשחייה או לשחק בקבוצות, קבוצה אחת בחולצות והשניה בלי חולצות.

ידוע שנערות ונשים סובלות משיפוט לגבי מראה גופן, גם מהחברה וגם מבפנים. גם נערים וגברים סובלים מזה.

כנראה טרנסים, הסובלים מדיסמורפיה מגדרית, שחווים את גופם כ"לא נראים כמו שהם צריכים להיראות", יכולים להזדהות עם המצב של הבנים כאלה.

אין לי שום ספק שהטיפול בגינקומסטיה "מאשר מגדר" לבנים הסובלים מזה במהליך ההתבגרות. לכן זו אני התאכזבתי מאוד, אך לא הופתעתי, ברגע שקראתי את המסקנה של "אפשרויות טיפול" בעמוד גינקומסטיה של שירות הבריאות הלאומי בבריטניה:

ניתוח הקטנת חזה לגבר אינו זמין בדרך כלל בשירות הבריאות הלאומי בבריטניה לטיפול בגינקומסטיה.

ההליך זמין בבתי חולים פרטיים ובמרפאות.

אני חיפשתי באתר קופת החולים שלי. מצאתי את מחלת גניקומסטיה. נראה לי שמבדילים בין סיבות רפואיות ל סיבות אסתטיות הטיפול, ובדיקה לטיפול הורמונלי קודמת לשאיבת שומן וניתוח נוסף. אני לא מצאתי הבדל כלשהו בין בנים המבקשים הסרת רקמת שד לא רצויה לכאלה המבקשים טיפול מגדר של נקבה לזכר כולל הניתוח המכונה "ניתוח עליון".

אני לא חיפשתי מספיק. אני אשמח לברר בוודאות אם יש הבדל כלשהו בחובה כספית או בהמתנה לקבלת הטיפול.

Jewish Press: On Final Days in Office Meretz Health Minister Pushes State-Funded Sex Change Operations

According to a Channel 14 report, outgoing Health Minister Nitzan Horowitz circulated a draft proposal instituting several radical policies under the noses of Israeli voters, calling for public funding for gender reassignment surgeries, including free breast removal surgeries for minors with the consent of their parents.

The draft was circulated by the medical division of the Health Ministry that was addressed to all hospital directors and the managers of HMO’s medical departments. It is signed by Dr. Hagar Mizrahi, head of the medical division at the health ministry. The draft was published on October 25 and requires a response from the addressees by November 16. This could mean that the content of the draft might be adopted by the ministry before a new coalition government is established.

The same draft also proposes allowing one to change one’s gender in the population registry (and consequently alter one’s ID card) only based on a statement, without having undergone the medical or surgical procedure to actually change one’s gender. (cont.)

Esser Agaroth (2¢):
The trend in the U. S. is to talk about "gender-affirming treatment," for minors who view themselves as trans-gendered, as opposed to just "treatment." 

I am all for "treatment," but that should begin with psychotherapy, and not an immediate prescription for hormones and the scheduling of body transforming, irreversible surgeries.

Even though Israel is more restrained, outgoing Israeli Minister of Health Nitzan Horowitz is still trying to advance an agenda, an agenda which, in my opinion, does not have the mental health of adolescents very high up on its list of priorities.

Horowitz's Meretz Party lost seats. So he will not be returning to the Knesset (Israeli Parliament) this coming term. His left leaning coalition will be in the opposition. So, it is no wonder that he is rushing around in a last ditch attempt to push through anything he can.

Upon hearing this euphemism "gender-affirming treatment," I immediately thought of the following condition.

Gynaecomastia (sometimes referred to as "man boobs") is a common condition that causes boys' and men's breasts to swell and become larger than normal. It is most common in teenage boys and older men....

Puberty
During puberty, boys' hormone levels vary. If the level of testosterone drops, oestrogen can cause breast tissue to grow. Many teenage boys have some degree of breast enlargement. Gynaecomastia at puberty usually clears up as boys get older and their hormone levels become more stable... 

Treatment options include

• medication to adjust a hormone imbalance
• surgery to remove the excess breast tissue

As far as I understand, gynaecomastia can sometimes be treated with liposuction alone, often a less complicated procedure than breast tissue removal and nipple modification in biological women, choosing to do so. The skin at that age can be elastic, and as a boy's pectoral muscles develop, he may not require any additional treatment.

Gynaecomastia can be devastating to boys, if they do not undergo treatment. Many boys simply try to hide their condition, terrified they will have to removed their shirts during physical education [PE] for swimming or to play on teams either as "shirts" or the dreaded "skins."

It is well-documented how girls and women incur judgments about the appearances of their bodies, both from society and from within. News flash! So do boys and men.

Trans people, suffering from gender dysmorphia, experiencing their bodies as "not what they are supposed to look like," should, themselves, be able to relate.

I cannot think of too many treatments, which are as "gender-affirming," as the treatment of gynaecomastia in boys going through the pain and awkwardness of adolescence. That is why I was disappointed, but not surprised, to see the conclusion to "Treatment options" on this NHS UK page:

Male breast reduction surgery is not usually available on the NHS for the treatment of gynaecomastia. 

The procedure is available at private hospitals and clinics.

I searched on my own Health Management Organization's [HMO] website in Hebrew. I did find gynaecomastia listed. It seems that a distinction is made between medical and aesthetic reasons for coverage, and that hormonal treatment may be indicated first, before liposuction and additional surgery. I did not find if there was, or was not, any difference in coverage for boys with unwanted breast tissue versus female to male gender transitional "top surgery."

I did not do a very thorough search. I would love to find out for certain if any differentiation is made in coverage or in the wait for such treatments.
